• Only submit this form if your email address or mobile number have changed since we last contacted you about the class action you registered in, or if you have not previously provided one.
• This form only applies to the Add-On Insurance Class Actions and Flex Commissions Class Actions settlements. You will be asked to choose which class action you want to update your contact details for.
• If you do not know which case you registered for, please check the case name on the notice we sent you before submitting this form. If we cannot identify your registration, we may not be able to update your details.
• This is not a registration form or contact form. We will not contact you about this request unless we need to clarify your contact details. New registrations will not be accepted for these cases pursuant to Court orders.
• We do not need your postal address. We will not send letters by post if we have a valid email address and/or mobile for you in order to reduce costs and delay.
• We will post on the relevant case's webpage when we contact registrants. Check there for regular updates to make sure you don't miss anything.
